As you consider treatment at Polaris, please take some time to review the following:

I am a resident of California or plan to be in the state of California for treatment.

Polaris Insight Center providers are licensed to work with patients in the state of California. Some out-of-state patients may choose to commute in and out of the state for treatment, but this requires a high level of coordination with the consistency of treatment and overnight stays.

If seeking KAP, I have reviewed the medical and psychological contraindications for KAP and none of the listed items apply to me.

Patients with one or more of the following medical contraindications and psychological conditions would not be a good fit for KAP Treatment at Polaris:

– Sleep apnea

– Untreated blood pressure issues

– Untreated heart conditions

– History of psychosis

– Stroke risk

– Active mania

– Recent TBI

– Untreated seizure disorders

– Recent suicide attempt (within the last 12 months) – You might be an appropriate candidate for KAP or IV infusions at another clinic with a higher level of care

– Recent intensive mental health treatment (within the last 12 months) – You might be an appropriate candidate for KAP or IV infusions at another clinic with a higher level of care

I understand that Polaris is an out-of-network provider.

Polaris is not contracted with any insurance company and does not communicate on behalf of clients with insurance companies.

KAP at Polaris is an out-of-pocket treatment. Patients may choose to submit superbills for reimbursement to their insurance company. We are happy to provide patients with procedure codes to get an estimate of what portions of treatment would be covered.
